Insurance & Payments
CCHC Nevada is currently accepting self-pay patients only and does not yet bill insurance plans. Below are the payment methods we currently accept:
- Cash
- Credit Cards (Visa, Mastercard, American Express, Discover)
- Debit Cards
- HSA (Health Savings Account) Cards
- FSA (Flexible Spending Account) Cards
We also welcome uninsured patients, offering a sliding fee discount and other affordable care options. Our staff can help you understand your coverage and available programs.
What to Bring to Your First Visit
To help us serve you as efficiently as possible, please bring the following:
- A valid photo ID
- Your insurance card (if applicable)
- A list of current medications
- Any relevant medical records or referral forms
- Payment method for applicable fees
Patients Forms
To help make your visit faster and more convenient, patients may complete required forms before arriving at the clinic. Completing forms in advance helps reduce wait times and allows our team to better prepare for your visit.
